Employee Near Miss Form

PLEASE NOTE - This is for reporting incidents where there was the potential for an employee injury only.

All patient or visitor incidents/potential incidents or misadminstrations should be reported through RL Solutions not the Employee Incident Reporting.

What is a near miss?

  • A near miss is an unintentional incident that could have caused damage, injury or death but was narrowly avoided.
  • In the context of safety, a near miss may be attributed to human error, or might be a result of faulty safety sytems or processes in an organization.
  • A near miss may also be called a close call, near collision or near hit.

Please go to the website, click on the blue button and follow the directions for logging into the Employee Incident Reporting System. Select Report an Event and then chose Near Miss/Hazard. Students and others who are not employees can use the the Student/Non-employee reporting form link found on the EHS web page for reporting employee injuries and illnesses.
