Supervisor Information
This page will help you determine which modules your staff need to complete. Begin with the Job Hazard Assessment. All staff must
complete Fire Safety, Hazard Communications, Emergency Preparedness, Ergonomics, General Work Surfaces and Electrical
Safety.
The training modules are based on typical job areas such as clerical staff or medical providers. You should review
these modules and instruct staff which additional presentations they need to complete.

The following documents must be maintained at each site as part of the Safety Desk Book:
- Chemical Inventory and corresponding Material Safety Data Sheets (MSDS)
- Job Hazard Assessment for each position at your location
- Training Records
- Attendance records for all training should be maintained for a minimum of 3 years
