Employee Incident (Injury/Illness) Reporting System

All patient or visitor incidents or misadminstrations should be reported through the Medical Center's Event Reporting System RL Solutions - https://sites.mc.rochester.edu/urmc-operations/quality-institute/healthcare-safety/event-reporting-system/. Please be sure you are using the right system to submit your report.

NOTE: AS OF APRIL 29 - The online system is temporarily unavailable due to a system wide problem. The software vendor is working on the issue. PLEASE SEE NOTE BELOW FOR ALTERNATE REPORTING PROCESS.

Using this Employee Incident Reporting system employees and supervisors can report:

  • work-related employee injuries and illnesses
  • near-miss incidents/hazards (An unplanned event that did not result in injury, illness, or damage - but had the potential to do so)
  • or request ergonomic assessments for computer workstations

When a work-related incident, injury or illness occurs, or you witness/are involved in a near-miss incident, please document the event by reporting it through this system. All events need to be documented within 24 hours of the occurrence if possible. Upon submission of the report, Leave Administration, Environmental Health & Safety, and the employee's immediate supervisor will be notified via email. All blood exposure needlesticks incidents (or injuries) should be reported the Blood exposure hotline at 275-1164 as well as through this on-line reporting system.

NOTE - While the on-line system is temporarily unavailable please call 275-3241 to submit an employee injury or illness report or if you are a supervisor trying to follow up on an incident. Someone will be available to assist you during normal business hours - MONDAY THRU FRIDAY, 8:00 AM - 5:00 PM. If you need to report an incident outside of those hours, please call and leave your name and a phone number where we can reach you during those hours or send an email to questions@safety.rochester.edu and we will get back to you. Thank you for your patience.
